BIRMINGHAM, Ala., Oct. 1, 2013 /PRNewswire/ -- SuccessEHS client First Care Clinic in Hays, Kan., recently received Level III PCMH recognition from The National Committee of Quality Assurance (NCQA). First Care Clinic was awarded recognition for using evidence-based, patient-centered practices that focus on coordinated care and integrating patients as active participants in their care. This highest level of recognition serves as confirmation of the clinic's efforts to provide high-quality, affordable care.
The PCMH care delivery model is designed to strengthen patient-physician relationships by moving from reactive care to proactive care, allowing a physician-led care team to facilitate an ongoing relationship for wellness and long-term healing. SuccessEHS provides EHR software and a comprehensive PCMH toolkit that serves a guide for clients as they transition to the PCMH model.
"SuccessEHS offers software with excellent reporting capabilities and helped us achieve PCMH recognition. We also would not have been able to achieve this goal without the active participation of our dedicated staff," said Rhiannon Maier, Clinical Director of First Care Clinic.
SuccessEHS's patient-centered EHR solution supports the PCMH model and includes robust reporting capabilities, an inherent patient portal, advanced health information exchange capabilities, tools for patient communication and advanced scheduling capabilities. SuccessEHS created pre-built reports specifically designed for clients pursuing PCMH recognition.
First Care Clinic has seen advances in its quality of care due to the implementation of the SuccessEHS EHR solution and their transition to the PCMH model.
"We've seen health improvements in our diabetic and hypertensive patients," Bryan Brady, CEO of First Care Clinic, said. "Our immunization rates for children under two have also significantly increased."
